2011 Residency Verification

Dear Irvine High School Community,

Click below to access printable versions of the Residency Verification Form and Residency Affidavit form that need to be completed and submitted to your student’s Teacher Advisor (TA) for all 9th, 10th, and 11th grade students.

Please submit these forms by Wednesday, April, 27th.  

You will need two bills attached to the Residency Verification.

Examples of acceptable bills:

Current Electric bill
Current Southern California Gas bill
Current Cable bill
Current Water bill
Current Waste Management Bill
Current Payroll Stub (both name and address must appear on payroll stub)
Current Property Tax or Income Tax Documents (from the IRS, State, or County)

Please remember to make a copy of both parts top and bottom, in English. 

Also, if you have not already brought in a copy of your student’s proof of their Tdap Booster immunization, please attach a copy to the Residency Verification documents.
